Description

A Lead Care Manager at an assisted living facility plays a pivotal role in ensuring the highest standard of care and well-being for residents. Responsibilities

• Oversee day-to-day operations of the care management team, ensuring quality care is consistently provided to all residents

• Supervise and train care staff, conducting regular performance evaluations and ongoing coaching to ensure high standards of resident care

• Coordinate with healthcare professionals, including physicians, nurses, and therapists, to ensure comprehensive care for residents

• Participate in resident assessments and care planning meetings, providing expert guidance and recommendations
