Description

The Resident Care Coordinator at an assisted living facility is responsible for overseeing and managing the overall care and wellbeing of residents. This role involves coordinating care plans, monitoring health conditions, and ensuring compliance with healthcare regulations. The Resident Care Coordinator works closely with caregivers, medical professionals, and the residents' families to establish personalized care strategies that promote quality of life and independence. Key responsibilities include assessing resident needs, organizing medical appointments, administering medications, and providing support for daily living activities. The position requires strong communication, organizational, and problem-solving skills to effectively address the diverse needs of the resident community.

Responsibilities

• Develop, implement, and monitor individualized care plans for each resident, ensuring that all needs are met in a respectful and efficient manner.

• Conduct regular assessments of residents' physical, emotional, and mental health and update care plans as needed to reflect any changes.

• Coordinate and supervise care staff, providing guidance and support to ensure that proper procedures and protocols are consistently followed.

• Serve as a primary point of contact for residents' families, addressing any concerns and keeping them informed of their loved ones' well-being and care.

• Ensure compliance with all local, state, and federal regulations pertaining to assisted living facilities and resident care.

• Organize and oversee the scheduling of medical appointments, transportation, and other essential services for residents.

• Facilitate effective communication among residents, healthcare providers, and care staff to promote a cohesive and collaborative care environment.

• Manage and maintain detailed and accurate documentation of each resident’s care, treatment plans, and progress notes.

• Plan and coordinate activities and programs that enhance the quality of life for residents, promoting socialization and engagement within the community.

• Conduct in-service training for care staff on best practices, new procedures, and regulatory updates to ensure the highest standard of care.

• Respond promptly to emergencies and incidents, taking appropriate action to ensure the safety and well-being of all residents.

• Ensure that the facility’s environment is safe, clean, and conducive to the overall health and comfort of residents.
