Description

The Resident Care Coordinator at an assisted living facility is responsible for overseeing and managing the overall care and wellbeing of residents. Responsibilities

  • Serve as a primary point of contact for residents' families, addressing any concerns and keeping them informed of their loved ones' well-being and care.

  • Ensure compliance with all local, state, and federal regulations pertaining to assisted living facilities and resident care.

  • Organize and oversee the scheduling of medical appointments, transportation, and other essential services for residents.

  • Facilitate effective communication among residents, healthcare providers, and care staff to promote a cohesive and collaborative care environment.

  • Manage and maintain detailed and accurate documentation of each resident’s care, treatment plans, and progress notes.

  • Plan and coordinate activities and programs that enhance the quality of life for residents, promoting socialization and engagement within the community.

  • Conduct in-service training for care staff on best practices, new procedures, and regulatory updates to ensure the highest standard of care.

  • Respond promptly to emergencies and incidents, taking appropriate action to ensure the safety and well-being of all residents.

  • Ensure that the facility’s environment is safe, clean, and conducive to the overall health and comfort of residents.
